If you reported your data last year, usernames and passwords are the same. If you have forgotten or don't know your username and password, or would prefer to have us mail you a paper copy of the survey, please contact us by e-mail or phone and we will provide it for you. E-mail is the preferred method of contact. If we are not in the office you can leave a message on our answering machine and we will return your call. We strongly encourage participants to complete the survey online. If this is not possible, you can find generic form and instructions attached below.  You can then mail it to us at:
